Prior to your first visit, please complete your online intake form and include your benefit/insurance information. Your intake form will be emailed to you once you have booked online. Please bring your Member ID Card and Policy or Plan Number with you to your first appointment.

In the case of most insurers, including ICBC, they pay up to a maximum amount per visit. The remainder is the responsibility of the patient. In the case of ICBC, we do not direct bill but you can submit your receipt to ICBC for reimbursement. Any remainder is eligible for submission to your private health insurer for consideration of coverage.
